Opt Out Form Login

If you are a Class Member and you want to “opt out” or ask to be excluded from this lawsuit and the Settlement, you must complete an Exclusion Request. If your Exclusion Request is properly completed and timely received, you will receive no benefits or payment from the Settlement, but you will retain your right to sue on your own.

To be effective and to exclude you from this lawsuit and the proposed Settlement, an Exclusion Request must be completed and submitted no later than October 23, 2026. All submitted Request for Exclusion Forms are subject to review and verification.

You can find your LoginID and PIN on the Notice you received in the mail. If you are unable to locate your ID and PIN, please contact the Settlement Administrator by emailing info@ZKBIPASettlement.com. Please request your ID and PIN by providing your full name and mailing address.

You may also mail in your Request for Exclusion by following the directions in Question 7 and sending your letter to:
